If you are unable to find a guarantor for your Canadian passport application, we will certify your Statutory Declaration in Lieu of Guarantor (Form PPT 132), passport photo, and copies of supporting documents (if you are mailing in your application). Please bring two forms of photo identification bearing your signature as we will need to confirm your identity and administer a solemn declaration regarding the contents of the statutory declaration.
